5/17/2023 EnchantaVaughn Approved 1.Behavior: Berando greeted QMHP-C as QMHP-C arrived to the home. Berando appeared to be in a positive mood while discussing with QMHP-C how his day was going. QMHP-C informed Berando that this session would include his mother at which mother discussed with QMHP-C on how Berando is struggling with managing his anger. 2. Behavior: Berando displayed off-task behaviors evident by him starring off into the wall and or being distracted by others surrounding him such as people entering in and out of the home. 3. Behavior: Berando displayed a poor attitude toward mental health professional due to personal conflicts and external distractions. Berando refuses to participate or contribute to group discussion. Berando constantly complains and make excuses for his behavior. Berando was seen preoccupied, as he was involved in his own feelings, thoughts, and activities. 1.Intervention: QMHP-C utilized open communication with Berando to discuss any feelings and thoughts that may trigger his anger aggression and cause him to act out inappropriately. QMHP-C encouraged Berando to communicate feelings of sadness, hurt, and anger. QMHP-C supported Mrs. Shabazz with understanding and to communicate the respect of Berandoâs feelings. 2. Intervention: QMHP-C assisted Berando with developing coping strategies when he is angered such as family involvement, peer interaction, music therapy and journaling. QMHP-C assisted Berando assisted in identifying positive and hopeful things in his daily routine at the present time. 3. Intervention: QMHP-C examined and evaluated Berando responses to feedback. QMHP-C focused on Berandoâs ISP objectives and what he needs to do in order to achieve them. QMHP-C offered new ways to heighten Berando awareness of his ill-mannered behaviors, as well as helping him with functioning more constructively within his daily life. QMHP-C encouraged Berando to present healthier habits and routine by challenging himself to make better choices and try new things. QMHP-C questioned Berando on the statements he made about having difficulty expressing his feelings. 1.Response: While exploring Berandoâs emotional state, Berando mentioned to QMHP-C that he was having a good day. Mother indicated to QMHP-C that she knows that Berando is not going to always do the right things because that is what children do. 2.Response: Berando discussed with QMHP-C that he does not do communicate like he should because if he did then he would have gotten his brother in trouble when he sent that picture but he didnât say anything because he did not want his brother to get in trouble. 3.Response: Berando was reluctant and hesitant. Berando did not offer any forms of social or friendly rapport back to QMHP-C, remaining unemotional displaying an unresponsive and nonchalant attitude throughout the session. Berando took over 20 minutes to reduce hyperactive behaviors. Berando became more impulsive and started to cause more disruptions. Berando explored numerous ways to express her anger and negative behaviors during the role play with QMHP-C. Berando stated his difficulties in certain situations but displayed a negative tone. Berando required assistance as he was unable to lower his voice.
